You will be connected with one of Dial Direct’s friendly and knowledgeable consultants, who will be able to guide you through the claim process and answer any questions you may have.

If the assured life passes away as a result of suicide or attempted suicide within the first 12 months from the policy commencement date, regardless of whether it was caused by temporary or permanent insanity, mental illness, or the influence of drugs or alcohol, the policy will not provide any cover or benefits.

Dial Direct only accepts claims that are submitted within six months of the assured individual’s death. When submitting a claim for funeral benefits, it is important to include several pieces of documentation. These include certified copies of the identity documents for both the life assured and nominated beneficiary, as well as proof of identity for each of them. The policy schedule and a fully completed DHA 1663 form (notification of death form) must also be included.  Proof of the beneficiary’s banking details and a bank statement from the past month should be submitted. In cases where no beneficiaries exist, a letter of executorship from the high court is necessary. If the death of the life assured was due to unnatural causes, an official police report will be required. Finally, the insurer may request any other documentary proof that they deem necessary.
